One article recommends oregano oil, then you scroll further, and another claims berberine is the missing piece. Someone in a Facebook group swears probiotics completely cured their gut symptoms, while the next person says probiotics made their bloating ten times worse.
So who should you believe?
The truth is, there isn't one magic or best supplement for SIBO.
Think of treating SIBO like repairing a house after a storm. If the roof is leaking, repainting the walls won't solve the problem. Likewise, if the plumbing is broken, replacing a window won't stop the flooding. Every tool has a purpose, but using the wrong one at the wrong time rarely leads to lasting results.
The same principle applies to SIBO.
Some supplements are designed to reduce bacterial overgrowth. While others help restore the natural cleansing waves of the digestive tract, known as the migrating motor complex (more on that later).
Certain supplements can ease the uncomfortable symptoms, such as bloating or abdominal pain, while others help replace nutrients that may have become depleted by bacteria that interfere with digestion and absorption.
Understanding the role each supplement plays is often far more important than simply chasing the strongest one.
In this blog post, I'll go through some of the best supplements for SIBO based on current evidence. You'll learn when different supplements may be helpful, how they fit into a comprehensive plan, and why no single product works for everyone.
Whether you're dealing with hydrogen-dominant SIBO, methane overgrowth (now called Intestinal Methanogen Overgrowth or IMO), or recurring symptoms after previous treatment, this article will help you make more informed decisions.
What is SIBO?
Small Intestinal Bacterial Overgrowth, or SIBO, refers to an abnormal increase or imbalance of microorganisms in the small intestine, an area of the digestive tract that normally contains far fewer microbes than the large intestine. (1)
Traditionally, SIBO has been described as an overgrowth of bacteria in the small intestine. However, the picture is a little more nuanced. In some cases, symptoms may be driven not only by the total number of microbes present, but also by the types of microorganisms involved, where they are located, and how they interact with digestion, gut motility, bile acids, and the immune system. When microbes in the small intestine ferment carbohydrates, they can produce gases such as hydrogen, methane, and hydrogen sulfide.
- Hydrogen is produced by certain bacteria.
- Methane is mainly produced by archaea called methanogens, which is why “methane SIBO” is now more accurately referred to as Intestinal Methanogen Overgrowth, or shortly IMO.
- Hydrogen sulfide is produced by specific sulfur-metabolizing bacteria and is increasingly recognized as another possible gas pattern, although testing availability and interpretation can vary.
These gases may contribute to symptoms such as:
- Bloating that tends to get worse as the day goes by,
- abdominal pain,
- excessive gas, belching,
- diarrhea, urgency, and/or constipation, incomplete bowel movements,
- food intolerances (esp. carbs),
- the uncomfortable feeling that digestion has become unpredictable,
- and skin issues, hair loss, low energy, and other symptoms may also appear. (3)
This is why the best supplements for SIBO are not the same for everyone.
Someone with hydrogen-dominant SIBO may need a totally different approach than someone with methane overgrowth, hydrogen sulfide patterns, constipation, diarrhea, or recurring symptoms after treatment.

Can supplements really help treat SIBO?
The short answer is yes, but they're rarely the ultimate answer, as many people tend to believe.
Many understandably begin searching for the best supplements for SIBO, hoping to find a natural alternative to antibiotics or a quick solution that finally ends years of digestive discomfort.
Unfortunately, recovery is rarely that straightforward.
SIBO isn't simply a bacterial problem.
In many cases, it's the result of an underlying issue that allowed bacteria to accumulate in the first place.
What underlying issues could these be? Reduced gut motility, previous food poisoning, abdominal surgery, adhesions, hypothyroidism, connective tissue disorders, certain medications, and other medical conditions can all increase the risk of bacterial overgrowth. (1)
If the underlying causes aren't addressed, even the most effective antimicrobial treatments may only provide temporary relief.
This is one of the reasons SIBO has a relatively high recurrence rate. Current research suggests that approximately 45% of patients experience recurrent SIBO after completing antibiotic therapy. (4)
Eliminating excess bacteria is only one part of the puzzle. Preventing them from returning often requires more:
- restoring healthy intestinal gut motility,
- supporting digestion,
- correcting nutritional deficiencies,
- supporting gut-brain communication,
- identifying the factors that contributed to the overgrowth in the first place.
With that said, supplements can play a valuable role throughout the healing process.
Several herbal antimicrobials have been studied as alternatives or adjuncts to prescription antibiotics. Certain probiotics appear helpful in specific situations, although they are not appropriate for everyone. Prokinetic supplements may help stimulate the migrating motor complex, while digestive enzymes and targeted nutrients can support recovery as the gut heals.
Rather than viewing supplements as a magic bullet, it's more helpful to think of them as members of a team. Each has a specific role, but no single player can win the game alone.
Understanding the different roles of the best supplements for SIBO
One of the biggest misconceptions surrounding SIBO treatment is the idea that every supplement should kill bacteria.
In reality, successful recovery usually involves several different therapeutic goals that work together over time.
The first goal is to reduce bacterial overgrowth.
This is where herbal antimicrobials such as berberine, oregano oil, allicin, and neem often come into the picture. These botanical compounds are intended to reduce excessive bacterial populations within the small intestine. Depending on the type of SIBO, they may be used instead of prescription antibiotics or as part of a broader treatment strategy.
However, eliminating bacteria is only the beginning.
Imagine pulling weeds out of a neglected garden without addressing why they grew there in the first place. If the soil conditions remain unchanged, the weeds are likely to return.
Your digestive system works in a remarkably similar way.
Between meals, the small intestine performs a natural housekeeping function called the migrating motor complex (MMC). These rhythmic contractions help move food particles, digestive secretions, and bacteria toward the large intestine. When this internal cleaning cycle slows down or becomes disrupted, bacteria have more opportunity to remain in the small intestine and multiply. (5)
For this reason, many functional medicine practitioners also recommend prokinetics, supplements, or medications that support normal gut motility, particularly after antimicrobial treatment has finished. (6)
Another important goal is restoring balance within the gut microbiome.
Although this may sound contradictory after trying to reduce bacterial overgrowth, rebuilding a healthy microbial environment is often an important part of long-term recovery. Finally, many people with SIBO benefit from nutritional support.
Because bacterial overgrowth can interfere with digestion and nutrient absorption, deficiencies in vitamin B12, iron, and fat-soluble vitamins are not uncommon, particularly when SIBO has been present for a long time. (7)
Other supplements, such as digestive enzymes or enteric-coated peppermint oil, don't directly eliminate bacteria but may help reduce symptoms and improve digestion while the underlying condition is being addressed.
You'll notice that the best supplements for SIBO don't all serve the same purpose. Some target the overgrowth itself, while others focus on creating an environment where your digestive system can function more effectively over the long term.
Understanding that distinction is one of the most important steps toward building a treatment plan that addresses not only your symptoms, but also the reasons they developed in the first place.
Best herbal antimicrobials for SIBO
When most people search for the best supplements for SIBO, they're usually thinking about herbal antimicrobials.
These natural compounds are commonly used to help reduce bacterial overgrowth in the small intestine and are often prescribed as an alternative to prescription antibiotics.
Herbal antimicrobials are intended to directly target the microorganisms contributing to SIBO. Think of them as the clean-up crew. Their goal is to help reduce the excessive bacterial population that shouldn't be living in the small intestine in the first place.
That said, no single herb works for every person.
The effectiveness of a particular antimicrobial often depends on whether you're dealing with hydrogen-dominant SIBO, methane overgrowth (Intestinal Methanogen Overgrowth or IMO), hydrogen sulfide, or a mixed presentation. Your medical history, symptom severity, previous treatments, medication use, and overall health also influence which supplements may be appropriate.
It's also important to remember that just because they count as natural doesn't mean they're automatically risk-free.
Herbal antimicrobials can interact with medications, cause side effects, and may not be suitable during pregnancy, breastfeeding, or certain medical conditions. For these reasons, it's always best to discuss supplementation with a qualified healthcare practitioner who understands these nuances.
With that in mind, here are some of the most commonly used and best-studied herbal supplements for SIBO.
Berberine
If there were an award for the most well-known herbal antimicrobial used for SIBO, berberine would almost certainly be a finalist.
Berberine is a naturally occurring compound found in several plants, including barberry (Berberis vulgaris), Oregon grape (Berberis aquifolium), and goldenseal (Hydrastis canadensis), as well as several others, including goldenthread (Coptis chinensis) and tree turmeric (Berberis aristata). (8)
It has been extensively studied for its antimicrobial, anti-inflammatory, and metabolic effects and has become a cornerstone of many herbal SIBO protocols. (9)
In functional medicine, berberine is most commonly used for hydrogen-dominant SIBO cases, in which excessive hydrogen-producing bacteria are thought to drive symptoms such as bloating, abdominal discomfort, loose stools, or diarrhea. One reason is that a study showed that IBS-D (diarrhea-dominant) patients who took 400 mg of berberine hydrochloride daily for 8 weeks experienced significantly reduced diarrhea frequency, urgency, and abdominal pain. (10)
Laboratory studies suggest that berberine has broad-spectrum antimicrobial activity against various bacteria. However, most clinical research has evaluated herbal combinations (e.g., FC Cidal, Dysbiocide, or Candibactin) rather than berberine in isolation.
One reason practitioners often include berberine in treatment plans is that it appears to have multiple mechanisms of action. Rather than targeting a single pathway, it may simultaneously influence bacterial growth, biofilm formation, and inflammation. (9)
While researchers are still working to understand these mechanisms fully, this broad activity makes berberine an attractive option within comprehensive herbal protocols.
In practice, berberine is rarely used as the only supplement. More commonly, it is combined with other botanical antimicrobials to create a broader therapeutic approach.
Although berberine is generally well tolerated, it isn't appropriate for everyone.
Some people experience side effects such as nausea, abdominal discomfort, constipation, or diarrhea when first introducing it, particularly if they begin with a high dose. (11)
Berberine may also interact with certain medications because it inhibits several liver enzymes. It may interact with medications used to manage blood sugar or blood pressure, cholesterol-lowering statins, heart rhythm stabilizing drugs, anti-anxiety medications, antidepressants, and other medications, so professional guidance is recommended. Berberine is also contraindicated in pregnancy and breastfeeding. (11)
Berberine has a short elimination half-life of roughly 3 to 8 hours, which is why dividing the daily dose helps maintain consistent concentrations in the gut throughout the day.
In general, the most studied dosing protocol is 500 mg three times daily with meals, for a total of 1,500 mg per day. (12)
In the BRIEF-SIBO trial, the protocol for uncomplicated SIBO cases used was 400 mg twice daily (800 mg total) for two weeks, which is now a commonly referenced benchmark for SIBO-specific use. (13)
If you are new to berberine, starting at a lower dose, such as 500 mg once or twice daily with food, gives your digestive system time to adjust and helps reduce the likelihood of nausea, cramping, or loose stools. These side effects are usually mild and tend to resolve on their own. If they persist, reducing the dose is typically sufficient. Because berberine's antimicrobial activity works locally in the small intestine rather than through systemic absorption, consistent daily use matters more than hitting an exact number.
Oil of oregano
Few supplements have gained as much popularity in the gut health world as oregano oil, and for good reason.
Oregano oil contains several active compounds, most notably carvacrol and thymol, which have demonstrated antimicrobial activity against a wide range of bacteria, fungi, and other microorganisms in laboratory studies. Carvacrol and thymol also show a synergistic additive effect when combined, meaning the mixture is more potent than either compound alone. (14)
For SIBO, oregano oil is frequently used as part of a combination herbal protocol rather than as a stand-alone treatment. It is generally considered one of the broader-spectrum herbal antimicrobials, meaning it may target multiple types of bacteria rather than focusing on a single species. (15)
Many people are surprised to learn that oregano oil isn't the same as the culinary herb sprinkled over pizza. Medicinal oregano oil is a concentrated extract, often standardized to contain a specific percentage of carvacrol. The quality and potency can vary considerably between products, which is one reason why choosing reputable supplement brands matters.
Because oregano oil is relatively potent, it can occasionally irritate the digestive tract and cause symptoms such as nausea, heartburn, or stomach discomfort, particularly when taken on an empty stomach.
It is also not intended for indefinite use. At therapeutic doses used in SIBO protocols (typically a daily dose of 200–600 mg of emulsified or enteric-coated oil over 4–6 weeks), it is reasonable to expect some degree of collateral impact on commensal bacteria (your good gut bugs).
The evidence suggests this is more of a reshaping than a wipeout, and the effect appears largely reversible. (16)
Still, most herbal antimicrobial protocols are designed to be used for a defined treatment period before being reassessed, rather than continued for many months without supervision.
Plus, it's worth mentioning that, similar to berberine, oregano oil (specifically carvacrol) has some evidence of liver enzyme inhibition, which means it can theoretically affect the metabolism of certain medications, including blood thinners, immunosuppressants, and certain antivirals.
Allicin
While hydrogen-producing bacteria are one form of microbial overgrowth, methane is a different story altogether.
Methane is produced primarily by microorganisms called methanogens, particularly Methanobrevibacter smithii, rather than bacteria. This is one of the reasons the condition previously known as methane-dominant SIBO is now more accurately referred to as Intestinal Methanogen Overgrowth (IMO).
For people with methane overgrowth, symptoms often include constipation, bloating, abdominal fullness, and slower intestinal transit. (17)
Because methanogens respond differently to treatment than many bacteria, herbal protocols frequently need to be adjusted accordingly.
One of the most commonly used supplements for methane overgrowth is allicin, the biologically active sulfur compound naturally found in garlic. (18)
It's important to clarify one common misconception: taking garlic capsules or simply eating more garlic is not the same as taking a stabilized allicin supplement.
Many commercial garlic supplements contain very little active allicin because the compound is unstable and breaks down quickly during processing.
This is why practitioners typically recommend stabilized allicin extracts that are specifically manufactured to preserve the active ingredient.
Although research on allicin specifically for methane overgrowth is still emerging, it has become widely used in functional medicine because of its antimicrobial and antifungal activity and clinical experience suggesting that it may help target methanogens when combined with other therapies.
In practice, allicin is often paired with additional herbal antimicrobials rather than being used alone, particularly in people with higher methane levels or mixed hydrogen and methane patterns.
Typical dosages involve 450–900 mg of allicin three times daily, yielding a daily total of 1,350–2,700 mg over a 4–6-week course.
Neem
Neem may not receive as much attention online as oregano oil or berberine. Still, it has been part of traditional Ayurvedic medicine for centuries and remains a valuable botanical within many SIBO protocols.
Extracts from the neem tree contain numerous biologically active compounds with antimicrobial, antifungal, and anti-inflammatory properties. (19)
Although high-quality human studies specifically evaluating neem for SIBO are still limited, many practitioners include it alongside other herbal antimicrobials, particularly for more stubborn or complex cases.
Like the other herbs discussed in this article, neem shouldn’t be viewed as a cure on its own. Instead, it often forms one piece of a carefully designed protocol that also addresses gut motility, nutrition, and the underlying drivers of bacterial overgrowth.
There are no standardized clinical trial dosages, so the typical dosage varies greatly; in general, 500 mg two to three times daily (1,000–1,500 mg/day), although dosing is product-dependent.
Botanical combination formulas
Interestingly, some of the strongest clinical evidence supporting herbal treatment for SIBO doesn't come from studies of individual herbs. Instead, it comes from combination formulas that combine several botanical extracts.
Products such as Candibactin-AR, Candibactin-BR, FC-Cidal, and Dysbiocide are commonly used because they combine multiple antimicrobial herbs into standardized formulations.
One frequently cited study, published by researchers at Johns Hopkins, directly compared these combination herbal products with rifaximin (the most commonly prescribed antibiotic for SIBO) in patients with confirmed SIBO.
The herbal group achieved breath test normalization in 46% of patients, compared to 34% in the rifaximin group. While the difference wasn't statistically significant, likely due to the study's size, the results support the conclusion that herbal combination therapy is equivalent to, and numerically slightly higher than, rifaximin. Notably, among patients who didn't respond to rifaximin and subsequently received herbal therapy, 57% went on to achieve a negative breath test, suggesting combination botanicals may also serve as an effective second-line option. Larger, prospective trials are still needed to confirm these findings.
The study used 2 capsules twice daily of each assigned formula for 4 weeks :
- Dysbiocide + FC-Cidal (Biotics Research pairing): 2 caps of each, twice daily
- Candibactin-AR + Candibactin-BR (Metagenics pairing): 2 caps of each, twice daily (20)
More recently, a 2024 study of 179 SIBO patients found that a comprehensive, personalized approach combining antimicrobial therapy, herbal supplements, probiotics, and dietary modification produced meaningful clinical improvement in over 72% of patients. This reinforces the idea that protocol design and individualization matter more than any single supplement or agent. (21)
This highlights an important point: successful SIBO treatment isn't usually about finding one magic supplement. It's about selecting the right personalized combination of therapies and using a holistic approach.
If you've been searching for the single best supplement for SIBO, it may be more helpful to shift your thinking slightly. The goal should be to build a protocol that targets the specific type of overgrowth you have while also supporting the digestive system as a whole and finding the root causes behind SIBO.
The best prokinetic supplements for SIBO
Many people assume that once the bacterial overgrowth has been treated, the journey is over.
Unfortunately, that's not always the case.
One of the biggest challenges with SIBO is its tendency to come back.
Studies suggest that recurrence is relatively common, particularly when the underlying cause hasn't been addressed. (4) This is why many practitioners don't consider antimicrobial treatment to be the finish line; it's simply one phase of the healing process.
Imagine you've finally cleared a traffic jam on a busy motorway. Everything starts moving again, but if the original accident remains on the road, it won't be long before traffic backs up once more.
Treating bacterial overgrowth without supporting gut motility can create a similar situation. You may temporarily reduce the bacteria, but if the digestive system isn't moving food and microbes efficiently through the small intestine, conditions remain favorable for overgrowth to return.
This is where prokinetics come in.
Prokinetics are substances that help support the normal movement of the digestive tract, particularly the migrating motor complex (MMC).
The MMC is sometimes described as the gut's housekeeping wave. Rather than helping digest food during meals, it works between meals and overnight, generating rhythmic contractions that sweep leftover food particles, digestive secretions, and bacteria from the small intestine into the large intestine. (22)
Think of it as the cleaning crew that arrives after everyone has left the building. If that crew never shows up, debris gradually accumulates. In the digestive system, that "debris" can include bacteria that would normally be cleared away before they have an opportunity to multiply.
Research suggests that impairment of the migrating motor complex is one of the major contributors to recurrent SIBO, particularly in people who developed symptoms following food poisoning. (23)
In these cases, antibodies produced after certain bacterial infections may interfere with the nerves that coordinate these cleansing waves, reducing the intestine's ability to clear itself efficiently. (24)
This helps explain why simply killing bacteria isn't always enough. Supporting gut motility after treatment may be just as important as reducing the overgrowth itself.
Ginger
When people think of ginger, they often picture a natural remedy for nausea or motion sickness. While it certainly has a long history in both traditional and modern medicine for relieving nausea, its potential benefits may extend far beyond that.
Ginger appears to influence several aspects of digestion, including gastric emptying and gastrointestinal motility. Some research suggests that specific compounds within ginger (gingerols and shogaols) may stimulate contractions within the digestive tract, making it one of the most commonly recommended natural prokinetics for people recovering from SIBO. Unlike stimulant laxatives, which primarily encourage bowel movements in the large intestine, ginger aims to support movement higher up in the digestive tract, where the migrating motor complex performs its important housekeeping function. (26)
Many practitioners recommend taking ginger after completing antimicrobial treatment rather than during the bacterial reduction phase, although protocols vary depending on the individual's clinical situation.
Most SIBO protocols commonly use 500–1,000 mg of ginger extract at bedtime to target the overnight fasting window, when Phase III MMC activity occurs.
It's also worth remembering that more isn't necessarily better. High doses of ginger may cause heartburn or digestive discomfort in some individuals, particularly those who are sensitive to spicy foods or who experience reflux.
Artichoke extract
Artichoke may seem like an unusual supplement for digestive health, but its role extends beyond simply supporting the liver or bile production.
Certain artichoke leaf extracts appear to stimulate upper gastrointestinal motility and may work particularly well when combined with ginger. Together, these two botanicals have become one of the most popular natural prokinetic combinations. (27)
Some practitioners recommend products that combine standardized ginger and artichoke extracts because they target different aspects of digestive function while complementing each other's actions.
Research using standardized combination extracts has used doses as low as 100 mg of artichoke and 20 mg of ginger extract per capsule taken twice daily, with studies showing meaningful improvements in gastric emptying and upper GI symptoms. (27)
Although research specifically examining these combinations in people with SIBO is still relatively limited, their growing popularity reflects both emerging evidence and extensive clinical experience.

Should you take probiotics if you have SIBO?
Few topics generate as much debate in the SIBO community as probiotics.
Spend just a few minutes reading online forums, and you'll quickly find completely opposite opinions. One person describes probiotics as life-changing, while another insists they made every symptom dramatically worse.
So which perspective is correct?
The honest answer is that both experiences can be true.
Unlike many other digestive conditions, probiotics for SIBO are far from a one-size-fits-all recommendation. Whether they are helpful depends on multiple factors, including the type of SIBO, symptom severity, the specific strains used, and where someone is in their treatment journey.
This is why blanket statements such as everyone with SIBO should avoid probiotics or everyone with SIBO needs probiotics are both oversimplifications.
The scientific evidence is also mixed.
Some studies suggest that selected probiotic strains may improve symptoms, reduce inflammation, or even support eradication when combined with conventional treatment. (28)
Other research shows little benefit, while some patients experience increased bloating, gas, or abdominal discomfort after introducing probiotics. Some evidence suggests that introducing certain probiotic strains while bacterial overgrowth is still active may worsen symptoms rather than improve them.
Specific Lactobacillus strains produce lactic acid and hydrogen as metabolic byproducts, which can add to the existing fermentation burden in the small intestine and potentially intensify bloating, gas, and brain fogginess in susceptible individuals. (29)
A study presented at the American College of Gastroenterology found that probiotic use, particularly with Lactobacillus-dominant products, was significantly associated with these symptoms among patients with measurable SIBO. This has led many practitioners to recommend waiting until after the antimicrobial phase before introducing probiotics, allowing the small intestine to be in a clearer state before reseeding begins. Part of this inconsistency stems from the fact that the term "probiotic" encompasses hundreds of different microorganisms. Judging all probiotics by the results of a single strain would be like judging every dog by the behavior of one particular breed.
Saccharomyces boulardii
One probiotic that often stands apart from the rest is Saccharomyces boulardii.
Technically, it isn't a bacterium at all; it's a beneficial yeast.
This distinction is important because antibacterial herbs and antibiotics target bacteria, not yeast. As a result, Saccharomyces boulardii can often be taken alongside antimicrobial treatment without being eliminated by the very supplements designed to reduce bacterial overgrowth. (31)
Research has shown that Saccharomyces boulardii may help support the intestinal barrier, reduce diarrhea, and modulate immune function. (32)
Some studies also suggest it may improve outcomes when combined with antibiotic treatment for SIBO. (33)
In my experience, it is often one of the better-tolerated probiotic options, particularly for people who struggle with loose stools or diarrhea.
However, even beneficial yeasts are not appropriate for everyone. Individuals with severely compromised immune systems, central venous catheters, or critical illness should avoid S. boulardii entirely, as it may contribute to severe fungal infection.
Bifidobacterium strains
Not all bacterial probiotics behave the same way.
Bifidobacterium species are often better tolerated compared to Lactobacillus strains because they are naturally more abundant in the large intestine rather than the small intestine (may theoretically carry a lower risk of fermentation) and produce various metabolic by-products (short-chain fatty acids (SCFAs)).
Specific strains such as Bifidobacterium infantis and Bifidobacterium lactis have been studied for their potential to support digestive health, regulate bowel habits, and reduce inflammation. (34)(35)
That doesn't mean everyone with SIBO should immediately start taking a Bifidobacterium supplement. Introducing probiotics too early, especially when symptoms are severe, may not always be well tolerated. Timing and individualization remain key.

Of 81 patients following this structured sequence, 60% reported moderate to great overall improvement at two months, suggesting that when timing is respected, Bifidobacterium strains can play a meaningful restorative role rather than simply adding more bacteria to an already disrupted environment. Lactobacillus reuteri
Among the Lactobacillus species, Lactobacillus reuteri has attracted growing interest due to its potential role in methane production.
Some research suggests that certain strains of Lactobacillus reuteri (Lactobacillus reuteri DSM 17938) may reduce methane levels and improve constipation. (37)
Interestingly, not all Lactobacillus species behave in the same way. While some strains may be helpful, others produce D-lactate, which has been hypothesized to contribute to symptoms such as brain fog in susceptible individuals. Although this remains an area of ongoing research, it highlights why selecting probiotics based on individual strains rather than simply choosing the product with the highest number of organisms is so important.

When should probiotics be introduced?
One question I hear frequently is whether probiotics should be started immediately after a SIBO diagnosis.
The answer depends on the individual.
Some practitioners prefer to introduce selected probiotics during antimicrobial treatment, while others wait until bacterial overgrowth has been reduced before gradually rebuilding the microbiome. Both approaches can be appropriate depending on the person's symptoms, previous responses to probiotics, and overall health picture.
If you've previously reacted badly to probiotics, it doesn't necessarily mean you'll never be able to tolerate them. Sometimes the issue isn't probiotics themselves but rather introducing the wrong strain, at the wrong time, or at too high a dose.
Ultimately, probiotics should be viewed as one tool within a broader treatment strategy and not as an essential requirement for everyone with SIBO.
Best supplements for SIBO symptom relief
While herbal antimicrobials and prokinetics are often the stars of a SIBO treatment plan, they aren't the only supplements that may be beneficial.
Many people with SIBO struggle with uncomfortable symptoms long before the bacterial overgrowth is fully resolved.
Persistent bloating, abdominal cramping, food intolerances, diarrhea, constipation, and fatigue can significantly affect quality of life. At the same time, bacterial overgrowth may impair digestion, damage the intestinal lining, and interfere with the absorption of important nutrients.
This is where supportive supplements come in handy.
Unlike herbal antimicrobials, these supplements aren't designed to eliminate bacteria. Instead, they help support digestion, calm symptoms, improve nutrient absorption, and create an environment that allows the digestive system to recover.
Although they may not seem as exciting as antimicrobial herbs, they are often the difference between simply surviving treatment and actually feeling better throughout the healing process.
Enteric-coated peppermint oil
Peppermint oil has been used for digestive complaints for centuries, and today it remains one of the most well-studied natural options for relieving symptoms associated with irritable bowel syndrome. (38)
Its primary benefit lies in its ability to relax the smooth muscle of the gastrointestinal tract. Imagine your intestines acting like a muscle that's been clenched for hours. Peppermint helps relax that muscle, which may reduce abdominal cramping, spasms, and the sensation of trapped gas.
Several clinical trials have shown that enteric-coated peppermint oil can improve symptoms such as bloating, abdominal pain, and overall IBS symptom severity. (39)
Because many people with SIBO also meet the diagnostic criteria for IBS, symptom relief from peppermint oil may be particularly valuable.
The enteric coating is important because it allows the capsule to pass through the stomach before dissolving in the small intestine. Without this coating, peppermint oil is more likely to cause heartburn or reflux, something many people with SIBO already struggle with.
Clinical trials have used 180-200 mg enteric-coated capsules, taken as 1-2 capsules three times daily, 15-30 minutes before meals, for 4 weeks. (40)
It's important to recognize, however, that peppermint oil doesn't treat the underlying bacterial overgrowth (even though it has documented antimicrobial properties against certain bacterial species). Think of it as turning down the volume on your symptoms while the real repair work is taking place behind the scenes.
Digestive enzymes
One of the less obvious consequences of SIBO is impaired digestion.
When bacteria begin fermenting food in the small intestine before your body has had a chance to properly digest and absorb it, meals can become more difficult to tolerate. Many people notice they feel excessively full after eating, experience bloating shortly after meals, or react to foods they previously tolerated without difficulty.
Digestive enzymes may help improve the breakdown of carbohydrates, proteins, and fats while the digestive system is under stress.
Most comprehensive digestive enzyme formulas contain amylase to digest carbohydrates, protease to break down proteins, and lipase to assist with fat digestion. (41)
Some formulations also include enzymes that target lactose or plant fibers, while others contain ox bile to support fat absorption.
It's important to understand that digestive enzymes don't kill bacteria or cure SIBO. Instead, they help your own digestive system do its job more efficiently.
Better digestion means less undigested food remains available for bacteria to ferment, which may help reduce symptoms in some people.
Not everyone with SIBO requires digestive enzymes, but they can be a useful short-term support tool, particularly for individuals with signs of poor digestion or pancreatic insufficiency. (42)
Interestingly, this relationship runs in both directions. Reduced pancreatic enzyme output means more undigested food reaches the small intestine, giving bacteria additional fuel to ferment, which can contribute to overgrowth in the first place. At the same time, SIBO itself can worsen pancreatic insufficiency symptoms, since bacterial overgrowth deconjugates bile acids needed for fat absorption, further impairing digestion downstream.
One prospective study found SIBO in 15% of chronic pancreatitis patients with pancreatic insufficiency, and specifically recommended considering SIBO testing in those whose digestive symptoms don't improve despite high-dose enzyme replacement. This bidirectional relationship is one reason enzyme support and SIBO treatment are often recommended together rather than as separate, competing explanations for the same symptoms. (43)
Partially Hydrolyzed Guar Gum (PHGG)
At first glance, recommending a prebiotic fiber to someone with bacterial overgrowth sounds completely backward.
After all, haven't many people with SIBO spent months avoiding fermentable fibers because they worsen bloating?
Surprisingly, PHGG is one of the exceptions.
Partially Hydrolyzed Guar Gum is a gentle, soluble fiber that has been studied alongside antibiotic treatment for SIBO.
Some research suggests that combining PHGG with rifaximin may improve bacterial eradication rates compared with rifaximin alone. (44)
PHGG work by selectively feeding beneficial bacteria (specifically Bifidobacterium) in the large intestine, thereby improving bowel regularity. (45)
Introducing PHGG too quickly can temporarily increase bloating and gas, particularly in people with severe overgrowth. This is why starting with a very small amount and increasing gradually as tolerated is a wiser approach.
In the study that found improved eradication rates, patients took 5 grams of PHGG per day for 10 days alongside their antibiotic. However, because SIBO patients are often more sensitive to any fermentable fiber, most practitioners don't start at this dose. A more typical approach is to begin with around 1 gram per day, then slowly increase every few days as tolerated, working up toward the 5-gram amount used in research over several weeks. This gradual build-up gives your gut time to adjust and makes it much easier to identify your personal tolerance threshold, rather than risking a flare that might put you off a genuinely helpful fiber for good. L-Glutamine
L-glutamine is often described as a gut healing supplement, but it's worth understanding exactly what that means.
Glutamine is an amino acid that serves as an important fuel source for the cells lining the intestinal wall. (47)
Although L-glutamine is frequently included in SIBO protocols, current evidence doesn't show that it directly treats bacterial overgrowth. Instead, its potential role lies in supporting the gut lining while other therapies address the underlying causes.
This distinction matters because many people expect glutamine to reduce bloating or eliminate SIBO itself. While some individuals report symptom improvements, these benefits are more likely related to improved gut barrier function than to antimicrobial activity.
It's worth noting that human trial evidence on glutamine's effect on gut barrier function is mixed; a 2024 review of 11 clinical trials found no consistent, clinically meaningful reduction in intestinal permeability, and most of the research was conducted in hospitalized patients rather than in people with functional gut conditions such as SIBO.
Most SIBO protocols recommend around 5 grams of L-glutamine, taken one to three times daily. Interestingly, this is well below the dose range (over 30 grams per day) where research has found even a modest, and somewhat questionable, effect on intestinal permeability. This gap between typical practice and the strongest available evidence is worth knowing, and it's a good reason to view glutamine as a gentle, supportive addition rather than a cornerstone of gut repair. Nutrient deficiencies common in SIBO
One of the most overlooked consequences of SIBO isn't the bloating.
It's what the bacteria may be stealing from you.
The small intestine is where most nutrient absorption takes place. When excessive bacteria begin competing for nutrients or interfere with normal digestion, deficiencies can gradually develop, sometimes without obvious warning signs.
This is one reason why some people with long-standing SIBO experience far more than digestive symptoms. Fatigue, hair loss, brittle nails, poor concentration, tingling in the hands or feet, muscle weakness, or reduced immune function may all reflect nutritional deficiencies that deserve investigation.
Vitamin B12
Vitamin B12 deficiency is one of the best-recognized nutritional consequences of SIBO.
Certain bacteria can consume vitamin B12 before your body has the opportunity to absorb it. Over time, this may contribute to fatigue, weakness, numbness or tingling, poor memory, mood changes, and certain forms of anemia. (49)
If blood tests confirm a deficiency, supplementation may be necessary. Depending on the severity, some individuals respond well to oral supplements, while others require injections prescribed by their healthcare provider.
It's worth noting that conventional labs typically flag B12 as deficient below roughly 200 pg/mL, whereas in functional medicine optimal levels are considered to be closer to 500-900 pg/mL. If your B12 falls in this gray zone, particularly between 200 and 500 pg/mL, asking your provider also to test methylmalonic acid (MMA) can help reveal whether a functional deficiency is present at the cellular level, even when serum B12 alone looks fine.
Iron
Iron deficiency is another common finding, particularly in people with chronic diarrhea, inflammation, or underlying conditions affecting the small intestine.
Low iron stores may contribute to persistent fatigue, shortness of breath during exercise, restless legs, hair loss, or reduced physical performance. (50)
Because excess iron can also cause problems, supplementation should ideally be based on laboratory testing rather than guesswork.
Standard labs often set the lower cutoff for women lower than for men, reflecting typical menstrual blood loss, but functional practitioners generally target a ferritin level of 50-100 mcg/L for both sexes once deficiency has been ruled out.
One important nuance for anyone with SIBO: because inflammation can artificially raise ferritin and mask a true underlying deficiency, some practitioners use a higher cutoff, closer to 70 mcg/L, before ruling out iron depletion in people with active inflammatory gut conditions.
Fat-soluble vitamins
Vitamins A, D, E, and K all depend on healthy fat digestion for proper absorption.
If SIBO interferes with fat absorption or if underlying conditions affecting bile flow or pancreatic enzyme production are also present, deficiencies in these vitamins may develop over time. (7)
Vitamin D deficiency is particularly common in the general population, so it's not unique to SIBO. Nevertheless, testing vitamin D levels may be appropriate, especially for people with chronic digestive disorders.
Zinc and magnesium
Although the relationship between SIBO and these minerals is less direct, inadequate dietary intake, diarrhea, malabsorption, and chronic inflammation can all contribute to low zinc or magnesium status.
Zinc plays an important role in immune function, wound healing, and maintaining the integrity of the intestinal lining, while magnesium supports muscle function, energy production, and normal bowel movements.
As with all nutrients, supplementation should ideally be guided by symptoms, diet, and laboratory assessment where appropriate.
Can you take too many supplements?
When people are desperate to feel better, it's understandable that they want to throw everything they can at the problem.
I've seen supplement cupboards that look more like small pharmacies.
At some point, it becomes difficult to know which supplements are helping, which are causing side effects, and which are simply adding unnecessary expense.
More supplements don't necessarily lead to faster healing.
In fact, introducing several new products at the same time can make it almost impossible to identify what's working. If symptoms worsen, was it the oregano oil? The probiotics? The digestive enzymes? Or simply the combination of everything at once?
Whenever possible, it's wiser to build a protocol thoughtfully and systematically, introducing supplements in a logical order while monitoring your body's response.
Quality is also far more important than quantity.
A carefully selected protocol containing five evidence-informed supplements is often far more effective than taking fifteen products recommended by strangers on social media.
Finally, remember that supplements are exactly what their name suggests: supplements.
They are designed to support your recovery, not replace a nutrient-dense diet, eating habits, adequate sleep, nervous system support, appropriate medical care, or addressing the underlying causes of your SIBO.
When these foundations are neglected, even the best supplements for SIBO can only do so much.
How to choose the best supplements for your type of SIBO
After reading about all of these different supplements, you might be wondering:
Okay, but which ones should I actually take?
It's a completely understandable question. Unfortunately, it's also one that doesn't have a simple answer.
The best supplements for SIBO depend on far more than a positive breath test. The type of gas produced, your symptoms, your medical history, previous treatments, medication use, nutrient status, and, perhaps most importantly, the underlying reasons the overgrowth developed all influence which supplements may be appropriate.
This is one reason why two people with SIBO can have completely different protocols and still both be following evidence-informed approaches.
If you have hydrogen-dominant SIBO
Hydrogen-dominant SIBO is often associated with symptoms such as bloating, abdominal discomfort, excessive gas, and diarrhea, although constipation can sometimes occur as well.
When bacterial reduction is the primary goal, herbal antimicrobials such as berberine and oregano oil are commonly included in protocols. These herbs are frequently combined with additional botanical extracts rather than used individually, allowing targeting of bacterial overgrowth through multiple mechanisms.
After the antimicrobial phase, attention often shifts toward restoring gut motility and reducing the likelihood of recurrence. This is where prokinetic support, alongside addressing any underlying contributors, becomes particularly important.
If you have methane overgrowth (IMO)
Methane overgrowth is now more accurately referred to as Intestinal Methanogen Overgrowth (IMO) because methane is produced primarily by methanogens rather than bacteria.

Supporting the migrating motor complex, restoring digestive function, rebuilding a healthy microbiome where appropriate, and correcting nutrient deficiencies may become more important than continuing antimicrobial herbs indefinitely.
This staged approach is one reason why successful SIBO recovery isn't simply about finding the strongest antimicrobial.
